Volunteer Waiver and Release of Liability

Volunteer Acknowledgement and Assumption of Risk

I, the undersigned volunteer, hereby acknowledge that I am voluntarily participating in the Halloween Community Volunteer Event organized by Canada Youth Fitness Alliance (CYFA). I understand that my volunteer duties may include, but are not limited to, event preparation and setup, assisting during the event, and post-event cleanup.
I understand and accept that participation may involve certain inherent risks, including but not limited to minor injuries, slips, trips, falls, or accidents. I acknowledge that I am physically capable of participating in these activities and assume all such risks voluntarily.

Release of Liability

In consideration of being allowed to volunteer at this event, I hereby release, discharge, and hold harmless Canada Youth Fitness Alliance (CYFA), its directors, officers, employees, agents, and volunteers from any and all claims, demands, damages, or causes of action arising out of or in connection with my participation, except in cases of gross negligence or willful misconduct.
I understand that I am serving as a volunteer without compensation, and I accept that CYFA does not provide insurance coverage for volunteers.

Consent for Minors (if volunteer is under 18)

I, the parent or legal guardian of the volunteer named below, give my permission for my child to participate as a volunteer in this event. I have read and understood the terms of this waiver and agree to its conditions on behalf of my child.
